康斯坦丁（奇诺•李维斯KeanuReeves饰）生来就能看出游走在人间的恶魔与天使，这令年幼时的他十分痛苦，曾经自杀想以这种方式结束自己的痛苦。后来获悉了上帝与撒旦之间有过协议，谁也不能直接插足人间，只能间接诱使人们选择天堂与地狱，而自杀者不得上天堂，他便开始利用自己天赋的超能力帮助铲除人间的恶魔，以此祈求获得一张天堂的门票。一次在帮助一名女警调查她的妹妹的离奇死亡事件时，康斯坦丁意外获释了天使长加百利意图和撒旦的儿子联手统治人间的阴谋。康斯坦丁当然要竭尽全力阻止这一黑暗降临人间……

该片改编自埃里希·玛利亚·雷马克撰写的同名经典小说，讲述在第一次世界大战期间，保罗·鲍曼和同学莱尔、米勒参加了德国国民志愿兵，被派往前线与法军作战。在战场上，他们目睹到了残酷的饥饿、血腥和死亡，战争的荒谬无情让保罗从迷恋战争到心灰意冷。

1996年11月30日公開。全国松竹系劇場にて上映。出崎統監督作品。OVAシリーズの流れを汲む作品で、原作にはない劇場用オリジナルストーリーとして製作したもの。音楽はOVA版KARTE6より参加した川村栄二が担当、主題歌は山根麻衣『InvisibleLove』。本作最大の特徴は、キャラクターも含めた作品そのものの米国風のデザインや、患者が血を吐くシーンや頭をぶつけるシーンなどに使われた、サーモグラフィー切り替えによる演出などである。当時の最新の技術を極限まで用いた演出と、出崎監督特有の「ハーモニー」や「陰影」、「入射光」などもあり、人間の「生死」や「苦悩」をリアルに演出している。なお、2005年末、手塚眞監督によって再び映画化された作品がブラック・ジャックふたりの黒い医者であるが、内容は本作とは全くの別物。なお、前述の2005年度に公開された映画の予告に於いて「初の映画化」と謳っているうえ、一般的なイメージとしてはこちらの映画の方が強いために、本作のイメージは薄れつつある（※公式サイトの豆知識にメディア化作品が記載されているこの作品もそれに記載されているため否定しているわけではない）。ただしDVDレンタルはされているため見ることができる。セルDVDでの発売もされているが、こちらは在庫切れが相次いで入手困難の状況にある。公開当年の1996年の12月、秋田書店からフィルムコミックが発売されている。全2巻。（1巻：ISBN4-253-17328-4、2巻：ISBN4-253-17329-2）1999年末、よみうりテレビ･日本テレビ系列にて地上波TV放送された。2008年12月31日（23時45分-25時30分）TOKYOMXにての地上波局としては異例の、年を跨いでのアニメ作品放映という形で実施された。
