某个下雨的日子，经营宠物医院的张文浩（李善均饰）带着还有一个月就要与之共赴婚姻殿堂的姜善瑛（金敏喜饰）前往老家看望父母。途中文浩下车购物，而善瑛在接了一通电话之后便从男友身边神秘失踪。焦急的文浩四处寻找，结果却发现一桩桩令他难以置信的事实。善瑛曾于2007年债台高筑宣布破产，而当年的照片与他失踪的未婚妻竟完全是两个人。这个失踪的女人真身为何？她的背后又隐藏怎样的秘密和故事呢？本片根据日本作家宫部美雪的同名原著改编，“火车”来自佛教用语，指载着作恶亡灵前往地狱的冒火的车子。《观无量寿经》云：“人以恶应堕恶道，命欲终时，地狱众火俱至，必有火车来迎。”

三十岁上的阿搏（周润发饰）心地善良，可惜事业一事无成，长年与痴迷茅山道术的表姐金花（叶德娴饰）相依为命，身边只有一个聋哑的昔日江湖兄弟“六尺三”跟随。某日，阿搏在一只古旧家具中发现女鬼魏小蝶（钟楚红饰）的求助信，遂奉献三年阳寿与小蝶结为鬼夫妻，以助其脱离阴婚羁绊。魏小蝶回阳世报恩，却被阿搏误会为落难大陆妹并再次搭救。阿搏女友阿美（王小凤饰）厌倦清苦生活频频出轨，终于弃阿搏而去，小蝶现身安慰，填补了阿搏的情感缺失，看穿小蝶女鬼身份的金花施法捉鬼，但终于被小蝶的诚意打动，反而帮助小蝶延长在阳世逗留的时间。阿美离开阿搏后生活并不如意，要求复合被阿搏拒绝后愤而自杀，化作厉鬼寻仇……

郑明宝（郑丹瑞饰）是香港某电台播报猪肉价格的DJ，他的朋友陈七本想托他走私药品，没想到郑明宝把事情搞砸，害得他锒铛入狱。郑明宝无意中得到了一把雨伞，并由此结识了冥间女鬼方茵（陶君薇饰），为此，郑明宝专门做了一档晚间鬼故事的节目，由方茵现身说法，收听率居高不下，连当红DJ蕾莎（顾美华饰）也对他另眼相看，暗送秋波。节目中，方茵讲述了自己的一段被负心汉欺骗、惨遭情敌迫害致死的过程。原来，她本是从南洋来香港接管广昌隆饭店的女继承人，偶然间邂逅了警察马光燊（吴大维饰），并堕入爱河。但是，马光燊其实是个花花公子，几次三番伤害了方茵纯洁的感情。特别是当他的情妇（青山知可子饰）登堂入室后，更是把伤心欲绝的方茵逼上了绝路……
